O‘TKIR TONZILLITNI DAVOLASHDA BOLALARDA INGALASYON BAKTERIOFAG TERAPIYASINI IMMUN TIZIMGA TA'SIRI

Mavzuning dolzarbligi. Bolalarda o‘tkir tonzillitni (O‘T) o‘z vaqtida tashxislash va davolash ambulator-poliklinik yordamining dolzarb yo‘nalishlaridan biri bo‘lib qolmoqda. Shu bilan birga, keyingi yillarda bakteriofaglar yordamida davolash usullarini joriy etish pediatriyada dolzarb tendentsiyaga aylandi. Tadqiqot maqsadi. O‘T bilan og'rigan bolalarning mahalliy immun tizimiga ingalyatsion bakteriofag terapiyasining (IBT) ta'sirini tahlil qilish. Tadqiqot materiallari va uslullari. Tadqiqot boshqariladigan randomizatsion usul yordamida o‘tkazildi. O‘T bilan og'rigan 4 yoshdan 15 yoshgacha bo‘lgan jami 212 nafar bola va 110 nafar amalda sog'lom bolalar (nazorat guruhi) tekshirildi. Birinchi guruhga standart umumiy davolanishni olgan O‘T bilan og'rigan 107 bemor bolalar kiritildi. Ikkinchi guruh 105 boladan iborat bo‘lib, ular standart davolanishga qo‘shimcha ravishda IBT olgan. Immunologik tadqiqotlar sIgA va TNF-a ni aniqlashni o‘z ichiga oladi. IBT pyobacteriophag-PLC (RF) yordamida amalga oshirildi. Natijalar. O‘T bilan og'rigan bolalarda kasallikning birinchi kunida sIgA darajasining pasayishi kuzatildi (kichik bolalarda - 40,9% gacha, o‘smirlarda - 41,9% gacha). Ikkala yosh guruhida davolanishning oltinchi kunida sIgA ning IBT bilan olgan bemorlarda - 96,9% gacha, IBT olmagan bemorlarga - 80,7% gacha o‘sishi qayd etildi. Xuddi shunday o‘zgarishlar TNF-a darajasini o‘rganishda ham kuzatildi. Kasallikning o‘tkir bosqichida yuqori bo‘lgan TNF-a darajasi davolanish davrida asta-sekin kamaydi. IBT olgan bolalarda davolanishning uchinchi kunida TNF-a boshlang'ich darajadan o‘rtacha 11,0% gacha, davolashning oltinchi kunida esa 17% gacha kamaydi. Xulosa. O‘tkir tonzillit bilan kassallangan bolalarni kompleks davolashda ingalatsion bakteriofag terapiyasidan foydalanish mahalliy immunitet ko‘rsatkichlarining 16,3% ga yaxshilanishi fonida, kasallikning klinik belgilarini 1,39 baravar ijobiy tamonga siljishga yordam berdi (p≤0,05).
