BEPULSIZLIK BO'LGAN AYOLLARDA PCOS XUSUSIYATLARINI RIVOJLANISHDA STEROID GORMONI DISFUNKSIYASINI REGULYATORLARI (CYP17A1-RS743572, CYP19A1-RS247015) DETERMINANT GENLARINI ROLI

Abstrakt

Xulosa. Maqsad: follikulogenez buzilishining molekulyar genetik prognozlariga asoslangan PCOSda bepushtlik bilan og'rigan ayollarda ART dasturlari natijalarining prognostik mezonlarini ishlab chiqish. Materiallar va tadqiqot usullari. Ishda belgilangan vazifalarni hal qilish uchun 125 nafar ayollar tekshirildi: birinchi guruh - 45 nafar birlamchi PCOS va bepushtlik bilan kasallangan ayollar; ikkinchi guruh - ARTga tayyorgarlik ko'rayotgan bepushtlik va PCOS bilan kasallangan 46 ayol; uchinchi guruh 26 shartli sog'lom ayollar. Yuqorida aytilganlarga asoslanib, biz laboratoriya ma'lumotlarini tahlil qilish asosida steroid gormonlarining holatini, genlarini (CYP17A1-rs743572, CYP19A1-rs247015) baholash bo'yicha o'z tadqiqotlarimiz ma'lumotlarini taqdim etdik. Natijalar va uning muhokamasi. CY19A1 rs2470152 geni polimorfizmining PCOS rivojlanishiga ta'siriga kelsak, mutant allellar bemorlarda nazorat guruhiga qaraganda sezilarli darajada yuqori ekanligi aniqlandi. Biz asosiy guruhni CYP19A1 bo'yicha MC (+) va MC (-) ga ajratganimizda va nazorat guruhi bilan solishtirganda, biz MC (+) va MC (-) da gomozigotli mutant genotipining ko'proq ekanligini aniqladik. guruh nazorat guruhi bilan solishtirganda. Bundan xulosa qilishimiz mumkinki, CYP19A1 genining homozigot mutatsion shakli PCOSda ishonchli induktsiyali rol o'ynaydi va bizning natijamiz sezilarli edi (chi2 - 5,74; asosiy guruhda p<0,02; chi2 = 5,2; p = 0,02 bemorlarda metabolik sindrom va chi2=3,9;metabolik sindromi bo'lmagan bemorlarda p<0,05). Biroq, tadqiqot PCOS rivojlanishida heterozigot genotipga induksiyalangan ta'sirni aniqlamadi (chi2<3,85; p>0,05). Shu bilan birga, yovvoyi tipdagi gomozigotli variant asosiy guruhda, shuningdek, MC (+) va MC (-) guruhlarida (OR≥0,5) PCOS ko'rinishida himoya rolini o'ynadi. Hardy Weinberg tenglamasiga kelganda, biz asosiy guruhda kutilgan va kuzatilgan natijalar o'rtasida sezilarli farq topmadik. Polimorfizmni bashorat qilish samaradorligini baholash, yuqorida aytib o'tilganidek, atigi 0,6 ni ko'rsatdi, bu prognoz samaradorligi mutant allel va genotip nuqtai nazaridan ishonchli emasligini anglatadi. Xulosa qilib aytganda, PCOS bilan og'rigan bemorlarda CY17A1 polimorfizmini o'rganishdan shuni aytish mumkinki, GG mutant genotipi bemorlarda nazorat guruhiga nisbatan statistik jihatdan sezilarli darajada keng tarqalgan. PCOS bilan kasallangan bemorlarni bo'lishda ular guruhlarga bo'lingan va nazorat guruhi bilan solishtirganda, MS + PCOS bilan kasallangan bemorlarda mutant shakl (GG) genotipi nazorat guruhiga nisbatan pastroq bo'lgan, ammo MS-PCOS bemorlarida nazorat guruhiga nisbatan, mutant gen ko'proq aniqlandi.
